Income Slips

  • T4 — employment income
  • T4A — pension, retirement, annuity or other income
  • T4E — Employment Insurance benefits
  • T4A(OAS) and T4A(P) — Old Age Security and CPP
  • T5 — investment income (interest, dividends)
  • T3 — trust income
  • T5008 — securities sold or redeemed
  • Rental income and expenses
  • Self-employment or business income and expenses

Deductions & Credits

  • RRSP and FHSA contribution receipts
  • Charitable donation receipts (cash and non-cash)
  • Medical expense receipts
  • Childcare receipts
  • Tuition receipts (T2202) and student loan interest
  • Moving expenses (for work)
  • Union or professional dues
  • Home office expenses, if applicable

Investments & Property

  • Investment statements and transaction history
  • Investment management or advisory fees
  • Capital gains or losses — purchase and sale details
  • Foreign property over $100,000 (needed for form T1135)

Life & Personal Changes

  • Prior year Notice of Assessment
  • Marital status changes
  • New dependants or changes in custody
  • Address changes
  • Disability Tax Credit approval, if applicable

Business & Corporate (if applicable)

  • Bookkeeping records or accounting statements
  • Bookkeeping software export (QuickBooks, Xero or similar)
  • Tax instalment payment history
  • T4 and T5 slips issued to employees or shareholders
  • GST/HST filing information and remittances
  • Corporate documents (articles of incorporation, annual return, share structure)

Please bring the above and all other relevant tax information. When in doubt, bring it along.
